    You can obtain loss runs from the agent, broker or carrier that provided you with coverage in the past. Loss runs provide an official record of your loss history. Most insurance carriers will require that loss runs be submitted for review in order to supply a quote or approve a quote to bind. If you choose not to supply loss runs for prior coverage, it could limit our ability to provide quotation.

      If the unlocked/unattended vehicle exclusion is not removed, the policy will not afford coverage in the event that equipment is stolen from a vehicle that is left unlocked or unattended. Whether this exclusion can be removed will depend on certain factors that will be addressed later in this application.
      Please note that the way you respond to this question could affect the deductible that is offered to you for coverage, the cost of coverage, or whether terms for coverage can be offered at all.

    • OWNED EQUIPMENT INFORMATION

      Please fill out this section of the application if you would like to be quoted for a policy that would cover equipment you own. Coverage will be afforded on an ANNUAL basis. If you are needing coverage for equipment that you are renting or borrowing from other people/entities, please fill out the rented equipment portion of the application.
    • If you had to replace every single piece of equipment you own, how much would it cost you to do so?
      The deductible is the amount of money you would be expected to pay out of pocket, should a claim be made on the policy. Please note that we will do our best to accommodate your requested deductible; however, there are some instances where it might not be possible. If the deductible is important to you, please be sure to look for it on your quote.
      This includes activities on boats, piers, in pools or any other type of situation where water has a much higher chance of damaging the equipment. THIS DOES NOT APPLY TO PADDLE BOATS, CANOES, SURF BOARDS OR ANY OTHER ELIGIBLE NON-MOTORIZED RECREATIONAL EQUIPMENT THAT IS MADE FOR THE WATER. PLEASE NOTE THAT MOTORIZED WATERCRAFT IS NOT ELIGIBLE FOR COVERAGE.
    • Equipment used for over and underwater activities is covered at a higher rate. Coverage is only provided if appropriate safety measures are taken to protect the equipment.
    • If you cannot provide these details, we will not be able to offer coverage for equipment you use over/underwater. Employing the use of food-grade Ziploc bags, condoms or other such rudimentary protections does not qualify as acceptable under coverage standards.
      Please note: not all carriers can provide coverage for drones or equipment harnessed to drones. We will notify you if drone coverage cannot be accommodated.
    • If the carrier cannot afford coverage for drones, this amount will be subtracted from the overall replacement value we'll quote.

    • Please provide an estimate of the amount of money you usually/intend to make annually renting out your equipment to others.
      This would provide coverage in the event that your equipment is damaged after you've loaned/rented out your equipment to a third party. It would not provide coverage if the person that you loaned/rented the equipment to never returned the equipment. If you want that type of coverage, you would need to request Voluntary Parting/False Pretense coverage. In order to be quoted for that coverage, you would have to request Rented to Others coverage as well.
    • It would be best to request the maximum replacement value you rent out to others at any one time.
      Voluntary Parting and False Pretense would provide coverage in the event that a person/entity you rent your equipment to, does not return it. This coverage requires that Rented to Others coverage be included in the quote.
      If you're unable to confirm this, we cannot offer voluntary parting and false pretense coverage. Any claims made against this line of coverage will require that you present a signed contract with the borrower/renter/lessee in order for the coverage to apply.
    • It would be best to request the maximum replacement value you rent out to others at any one time.
      The schedule should list the make, model, serial number (if applicable) and replacement value of each item you would like the policy to cover. Some carriers will require a schedule in order to offer terms, so it would be beneficial to our search if you could supply a schedule.
    • Accepted file types: jpg, gif, png, pdf, xls, xlsx, doc, docx, Max. file size: 50 MB.
      Items worth more than $5K MUST be scheduled for coverage. If you're not able to supply a comprehensive schedule of equipment, you will need to advise additional information for the items that are worth more than $5K in order for us to assure proper coverage.

      Trailers must be used for business purposes. Motorized vehicles would be excluded from coverage.
      Business income insurance replaces the income your business loses when you can’t operate because of a covered property loss. Extra Expense coverage pays for the additional costs you incur to keep the business running after a covered loss.
    • This would typically cover: Lost net income, Continuing operating expenses (rent, utilities, payroll), Loan payments, Taxes
    • Extra expense would help pay for: Renting a temporary workspace, Leasing equipment, Paying overtime to speed up recovery, Expediting shipping or repairs
      IF YOU HAVE A BUSINESS AT A FIXED LOCATION, THIS WILL COVER IMPROVEMENTS YOU HAVE MADE TO THE BUSINESS SPACE. EXISTING IMPROVEMENTS INSTALLED BY A BUILDING OWNER OR PREVIOUS TENANTS WOULD BE EXCLUDED. IF YOU DO NOT OPERATE AT A FIXED LOCATION, THIS COVERAGE WOULD NOT BE BENEFICIAL TO YOU.
      If your business does not have a fixed location from which it operates, then tenant improvements & betterments coverage is not going to benefit you.

      Please fill out this portion of the application if you are looking to rent equipment from others. Rented equipment coverage would cover rented AND borrowed equipment.
      If you are also requesting a quote for owned equipment coverage, we would recommend that you select "annual", especially if you think you will be renting equipment multiple times during the upcoming year.
    • PLEASE NOTE: Rented equipment coverage does not provide coverage for VEHICLES.
    • Please enter a number less than or equal to 1000000.
      This is NOT the cost of the rental. It is the amount of money that the rental vendor would expect you to pay them if all the equipment you're renting disappeared. PLEASE ALSO NOTE: Rented Equipment coverage does NOT provide coverage for vehicles. Please do not include vehicle values with this figure.

      IF YOU HAVE A CLAIM, CONTINUING RENTAL FEES COVERAGE CONTINUES PAYING THE RENTAL FEES DUE TO THE RENTAL HOUSE/INDIVIDUAL YOU RENTED FROM, UNTIL THE CLAIM IS CLOSED. THIS IS SUBJECT TO A 72-HOUR WAITING PERIOD FROM WHEN THE RELATED CLAIM IS REPORTED.
    • Waterproofing equipment in things like food-grade Ziploc bags or condoms will not be approved by underwriting. The waterproofing methods must be implemented with professional grade elements.
      PLEASE NOTE: Coverage cannot be afforded when taking equipment to foreign countries that the United States currently has economic sanctions against. These countries include, but are not limited to: Venezuela, North Korea, Iran, Cuba, Syria & Russia.
      AS A REMINDER: rented equipment coverage does NOT provide coverage for vehicles. Please do not include value of vehicles when responding here.
    • You can provide this information later; however, it will be required if you decide to bind coverage, and could impact the cost of the policy.

    • Please note: the underwriter will only allow equipment to be stored in vehicles overnight if the vehicles are stored in approved, monitored facilities; or if they are parked in a private, residential garage that is only accessible to the insured and production personnel. The underwriter will not allow for equipment to be stored overnight in vans, trucks, buses or vehicles parked in areas that are at all accessible to members of the general public.
      If you're leaving the equipment at a commercial facility or shoot location overnight, and a member of production is not stationed with the equipment while it's there, that is an example of the equipment not remaining in your care, custody and control until it's returned to the rental vendor.
