COVERAGE OPTIONS
Providing copies of these documents will ensure that the quote we provide meets the requirements of your locations, vendors, permit offices, etc. We can also review the requirements and advise you of any waivers you should request to save yourself some money.
Please Note: The $1,000,000/$3,000,000 option is only offered by select carriers. If you choose that option, depending on other requests, we might have to include Excess Liability to reach that limit.
This is the maximum amount of money that the carrier will pay (usually $5,000) to members of the general public who are injured due to the production's fault or negligence, without a lawsuit being filed. Some carriers include this and others charge a fee for it. THIS WILL NOT COVER INJURY TO CAST AND CREW.
This is not common and incurs an additional premium. A WOS prevents your insurance provider from trying to recoup money from an additional insured's insurance provider in the event that a claim is paid out on your policy.
This is not common and usually increases the cost of the policy. Normally only required when a location or permit office require it.
This is ADDITIONAL liability that goes on top of the general liability for higher limits. This is not normally required by most locations, vendors or permit offices
AUTO COVERAGE
This covers auto liability for any vehicle that is not owned by the Named Insured for bodily and property damage. This does not cover damage to the vehicle itself.
Borrowing vehicles can mean a couple of things: you're hiring contractors, cast and/or crew that will be using their vehicles to execute activities for the production; or you're actually asking someone to use a vehicle they own, even though that person may not be working on the production.
This covers the rented vehicles themselves. If you are renting a grip truck or other vehicle and need full coverage, this must be included
Please select NO if you will be valeting the vehicle, storing it at a secured facility or returning it to the rental vendor or owner once you're done filming for the day.
INLAND MARINE COVERAGE
Rented Equipment covers equipment that is NOT owned by the Named Insured. It can be rented or borrowed from others (such as rental houses, directors of photography, cinematographers, etc.). Rented Equipment covers grip, lighting, cameras, lenses and other equipment used for production that is not a prop, set or wardrobe.
Owned Equipment coverage can be provided on a short term basis by select carriers; however, it will significantly increase the cost of the policy we'd be able to offer. Please note that we will attempt to get you the deductible you request; however, the quote we offer will first and foremost reflect the most affordable option for coverage we can provide. If the deductible is important to you, please be sure to look for it on the quote we remit.
Equipment stolen from an unlocked or unattended vehicle is excluded by default with most carriers. Depending on who we end up quoting you with, we could need additional information to remove this exclusion.

If the equipment is not going to be with the insured from the time it leaves the rental house until it is returned to the owner or rental vendor, then you should answer NO to this question.
This provides coverage against direct physical loss, damage, or destruction of digital media, raw film or tape stock, exposed film (developed or undeveloped), recording videotape, sound tracks and tapes, and loss due to faulty equipment, development, editing, processing or accidental erasure of recording up to the amount of the insured production cost. Not all carriers offer this coverage on a short-term basis, and it will add at least a few hundred dollars to the cost of the policy.
Third party property damage covers damages to permanent structures of locations (i.e. walls, doors, railings, windows, floors, etc.). It would not provide coverage for things like furniture, decorations, rugs, window dressings or other things at a location that might not be a part of the permanent structure of the building. If you need that kind of coverage, please request props, sets and wardrobe coverage.
Extra Expense covers additional costs that the production incurs due to a covered loss under the policy. For instance, if equipment you rent is damaged and you have to pay to rent new equipment, the policy would reimburse you for the cost to re-rent equipment.
Please Note: CAST COVERAGE DOES NOT COVER ON-SET INJURY TO CAST AND CREW (if you're looking for that kind of coverage, you'll need to request either Worker's Compensation or Accident Medical Coverage). It provides coverage for extra expenses incurred by the production due to the absence of cast or crew members because of death, disability (accident or sickness) or kidnapping. Cast coverage provides protection from pre-production through production and post-production. All cast and crew must be scheduled in advance and medical exams are required for sickness coverage. For coverage on an unscheduled basis, you may request the 'covered person extension WITH or WITHOUT sickness coverage'.

Please Note: THIS DOES NOT PROVIDE COVERAGE FOR ON-SET INJURY TO CAST AND CREW. It provides coverage for extra expenses incurred due to the accidental injury, death, sickness or kidnapping of a cast/crew member without having to schedule each individual for the coverage. The extension expands the definition of "covered person" to include any person necessary for the completion of the insured production. A medical exam will need to take place and be submitted to the underwriter prior to a claim being made, in order for sickness coverage to apply.
Please note: THIS DOES NOT PROVIDE COVERAGE FOR ON-SET INJURY TO CAST AND CREW. PLEASE ALSO NOTE: If cast and crew are to be covered for sickness, medical exams for those individuals will need to be performed and submitted to the underwriter BEFORE a claim is made.
Expands cast coverage to provide for a bereavement period up to 5 days due to the death of an immediate family member.
Provides reimbursement for extra expenses incurred by the production should an animal that is being used be unable to continue production due to illness, injury or death. This would not provide reimbursement for treatment or monetary compensation for death, should an animal's illness, injury or death be attributable to the production. That kind of coverage is supplied by requesting Animal Coverage.
COVERAGE FOR INJURY TO CAST & CREW
Please NOTE: We can offer either Worker's Compensation OR Accident Medical - but not both.
Worker's compensation covers the cast and crew for illness, injury or death that is attributable to working on the production. Worker's compensation CANNOT be offered to productions that will be filming stunts, fight scenes, farm/wild animals or other hazardous activities. If you will have any of those types of activities in your production, you will need to seek worker's compensation from a payroll company. Payroll companies do offer work comp for most productions; however, you would need to use their payroll services to qualify for the coverage. Worker's compensation cannot be offered to residents of North Dakota, Ohio, Washington & Wyoming.
You MUST eventually provide an FEIN if you're an LLC, LLP, Non-Profit or Corporation. The only time worker's compensation can be bound using only a social security number is if an individual or DBA is applying for the coverage.
PLEASE NOTE: Including owners/operators/officers in the worker's compensation coverage will significantly increase the cost of the policy, if we're able to offer the option.
Please note that we will try to provide this coverage, if we can, but there are some states/providers that do not offer it.
If you need the policy to cover independent contractors, please include their cost of hire in this figure. If your cast and crew are on deferred pay status, please provide the figures that you hope to eventually pay them. DO NOT include the payroll for owners/officers/operators in this figure.
Accident/Medical provides reimbursement for medical expenses due to on-set injury. We cannot offer this coverage if you're going to have Workers' Compensation coverage in place (whether it's placed through us or someone else). Accident/Medical is NOT an acceptable substitute for worker's compensation. So, if you're being required to have worker's compensation, please do not request this coverage. This is simply a layer of protection for cast and crew for those productions choosing not to purchase worker's compensation. PLEASE NOTE: If you do not see an option to be quoted for Worker's Compensation, it is because you have indicated that you will be filming certain things that would preclude us from being able to offer the coverage.
This does not include deferred pay or volunteer work for film credit. Only if you are actually paying the cast and/or crew.
This would provide reimbursement for treatment of an animal or a monetary death benefit if the production is responsible for the injury or death of an animal while it's being used for production. Sickness coverage can be included if a certificate of health by a duly licensed veterinarian is submitted to the underwriter BEFORE a claim is made. Requires that we include Animal Extra Expense Coverage.
